How To Use The Mounjaro KwikPen: A Step-By-Step Guide

Injecting yourself with the Mounjaro KwikPen can be frightening for some, although it doesn’t have to be. Mounjaro is a very effective and popular weight loss treatment for those with obesity or who are overweight with weight-related health problems.

The best way to ensure the Mounjaro KwikPen works as intended is to use it correctly. This step-by-step guide will give you information on how to use the Mounjaro KwikPen, giving you confidence to use yours easily, confidently and without hassle.

What is the Mounjaro KwikPen?

Mounjaro contains the active ingredient tirzepatide. This medication is used to treat adults with type-2 diabetes as well as weight loss and weight management in adults.

Tirzepatide is both a GIP and GLP-1 analogue that regulates blood sugar levels (glucose levels) in adults with type 2 diabetes, obesity, or who are overweight. This medication helps control cravings by reducing the feeling of hunger and replacing it with the feeling of fullness.

The Mounjaro KwikPen comes pre-filled with 4 doses, taken once weekly, lasting 4 weeks. The KwikPen consists of the following parts:

  • Pen cap
  • Pen clip
  • Red inner seal
  • Cartridge
  • Plunger
  • Cartridge holder
  • Pen
  • Label
  • Dose indicator
  • Dose window
  • Dose knob
  • Outer needle shield

Step-by-Step Guide on How to Use the UK Mounjaro KwikPen

Assembling the Mounjaro KwikPen

  • Step 1: Before handling your KwikPen, thoroughly wash your hands with water and soap.
  • Step 2: Take the pen cap straight off the pen. Look at the pen label, making sure the medication name and dose match your prescription, the pen is not damaged, it has not expired, and the medication has not been frozen. The medication should be colourless to slightly yellow.
  • Step 3: Use a clean swab to wipe the red inner seal of the pen.
  • Step 4: Choose a new needle for the pen. Always use a new needle each time you inject a dose of Mounjaro. This helps prevent any infections and blocked needles. You must then pull off the paper tab on the outer needle shield.
  • Step 5: Push the capped pen needle onto the KwikPen and twist it until it is secured tightly.
  • Step 6: Remove the outer needle shield and store it somewhere safe; you will reuse it later. Then, remove the inner needle shield and throw it away.
  • Step 7: Carefully turn the dose knob until you hear 2 clicks and the extended line is visible in the dose window. This is called the prime position. If you miss it, you can correct it by turning the dose knob in either direction until you see the prime position in the dose window.
  • Step 8: Hold the KwikPen with the needle pointing up. Gently tap the pen cartridge holder to move any air bubbles to the top of the cartridge.
  • Step 9: Still holding the KwikPen with the needle pointing upwards, push the dose knob in until it stops, releasing some of the medication into the air. Count to 5 seconds while holding the dose knob. The [-0] icon should now be visible in the dose window. This step is called priming the injection.

Priming removes air bubbles from the cartridge and ensures the KwikPen works correctly. If a small amount of medicine comes out of the tip of the needle, the KwikPen has been adequately primed.

If you do not see any medicine come out, repeat steps 7-9 no more than 2 times. If you see no medicine after 2 additional times, change the KwikPen needle and repeat steps 7-9.

How to Inject With the Mounjaro KwikPen

After assembling your KwikPen, you are ready to administer your dose.

  • Step 1: Choose an injection site. Mounjaro is injected under the skin of the upper arm, upper thigh (leg), or in the stomach area. Change or use a different injection site each week to avoid hard lumps forming.
  • Step 2: Twist the dose knob until it stops on [-1] in the dose window. The [-1] icon indicates a total 0.6 ml dose.
  • Step 3: Insert the needle into your chosen injection site.
  • Step 4: Inject the dose by pushing the dose knob in until it stops. Count to 5 while continuing to hold the dose knob. Before removing the needle, ensure the [-0] icon shows in the dose window.
  • Step 5: Remove the needle from your skin. It is normal for a small drop of medicine to be on the tip of the needle. If you do not see the [-0] icon in the dose window, insert the needle back into your skin to finish the injection. Do not redial the dose.

After the Mounjaro KwikPen injection

  • Step 1: If there is blood at the injection site, lightly press the site with a cotton ball or gauze until the bleeding stops. Do not rub the injection site, as this may cause further bleeding.
  • Step 2: Carefully replace the outer needle shield over the needle.
  • Step 3: Unscrew the capped needle and place it in your designated sharps container. Do not put the KwikPen back into the fridge with the needle still attached. This is to prevent air from entering the pen and any leaking or blocking the needle.
  • Step 4: Place the pen cap back onto the KwikPen. Ensure the pen is not stored without the pen cap.

Storage Instructions for the Mounjaro KwikPen

Used KwikPens

After using your Mounjaro KwikPen for the first time, you can store it at room temperature up to 30℃. Make sure you keep the pen away from heat, such as the sun, stove, oven, etc. Throw away your KwikPen 30 days after first using it, or dispose of the pen after using the 4 weekly doses.

Ensure that you keep the pen and needles out of reach and sight of children.

Unused KwikPens

Store your unused Mounjaro KwikPens in the refrigerator (between 2℃ to 8℃) until the pen expires. Do not place your pen in the freezer. If your pen has been frozen, do not use it.

Disposal of the Mounjaro KwikPen

Once you have injected your weekly dose of Mounjaro, throw away the pen needle in your sharps disposal container directly afterwards. Do not place the needles in your household waste.

Needles in household or general public waste pose a risk to contractors or others (needlestick injuries). If you need a sharps bin or are unsure about disposing of your needles responsibly, please contact us.

Sharp bins are classified as clinical waste and require special arrangements to be collected and disposed of. To see if your council will collect your sharps bin, you can check your postcode here (UK and Wales only).

You can throw away your used KwikPen (without the needle) in either your household waste or in the sharps container. The Mounjaro KwikPen is not recyclable.

If the pen still has medication in it, ask your healthcare provider or Cloud Pharmacy pharmacist how to safely dispose of it.

Troubleshooting Tips for the Mounjaro KwikPen

Sometimes, things go wrong, even after following each step. Below, we have included common issues experienced when using the Mounjaro KwikPen. If you need further assistance, please contact us.

Not Changing Needles Each Time you Inject

Needles are intended to be single-use, meaning that if they are reused, the lumen of the needle is damaged. This increases the risk of not getting the correct dose of Mounjaro and experiencing injection site reactions, which are small, hard fat lumps that form under the skin.

Not Checking the Expiry Date or Medication Window Before Injecting

Taking expired medication can cause adverse reactions, as well as not being effective. Before using your KwikPen, check the expiration date.

Mounjaro should be colourless to slightly yellow. If you look in the medicine window and the liquid is cloudy, discoloured or has visible particles, do not inject this medication.

Injecting Mounjaro Straight Out of the Fridge

You can use your Mounjaro KwikPen straight out of the fridge. However, cold injections will sting more than those administered at room temperature.

Not Holding Down the Dose Button for the Full 5 Seconds

After inserting the needle into the injection site, you need to push down the dose button until it stops. Slowly count to 5 while continuing to hold the dose button down.

If you let go earlier or do not wait the full 5 seconds, you will not receive the correct dose. Before removing the needle, ensure the [0] icon is shown in the dose window.

Forgetting a Weekly Dose

If you forget to take a dose, please contact us for advice on the next steps. A medicine reminder app is a helpful way to remember weekly doses.
