Managing Pain with Over-the-Counter Treatments
Pain is one of the most common reasons people seek treatment from a pharmacy. Whether it is a headache, muscle ache, joint pain, period pain, toothache or back pain, there are effective over-the-counter options available to help manage symptoms. Choosing the right type of pain relief depends on the nature, location and severity of the pain, as well as any other medicines you are taking and any underlying health conditions.

Who Are These Treatments For?
These products are suitable for adults and, where indicated, children. They may be appropriate for:
- Headaches, including tension headaches and migraines
- Muscle aches, strains and sprains
- Back pain and joint pain
- Period pain (dysmenorrhoea)

- Arthritis and rheumatic pain
- Post-exercise soreness
If pain is severe, sudden, persistent, or accompanied by other symptoms such as chest pain, numbness, weakness, or unexplained weight loss, seek medical attention. Persistent or recurring pain may indicate an underlying condition that requires investigation.
Important Safety Information
- Aspirin should not be given to children under 16 years of age
- Ibuprofen should be taken with food to reduce the risk of stomach irritation. It is not suitable for people with a history of stomach ulcers, kidney problems, or certain heart conditions
- Do not take more than one paracetamol-containing product at the same time - many combination products (cold and flu remedies, co-codamol) already contain paracetamol
- Co-codamol contains codeine, which can cause dependence if used regularly for more than three days. Do not use for longer than directed without medical advice
- Topical pain relief gels (e.g. Voltarol, Biofreeze) should not be applied to broken skin, eyes, or mucous membranes

Available Treatment Types
Paracetamol
Paracetamol is the most commonly used pain relief medicine in the UK. It is effective for mild to moderate pain and for reducing fever. It is generally well tolerated and can be taken by most adults and children (in appropriate formulations). The standard adult dose is 500mg–1g (one to two tablets) every 4–6 hours, up to a maximum of 4g (eight 500mg tablets) in 24 hours.
Ibuprofen and Anti-Inflammatory Painkillers
Ibuprofen is a non-steroidal anti-inflammatory drug (NSAID) that reduces pain, inflammation and fever. It is particularly effective for pain associated with inflammation, such as muscle strains, sprains, joint pain and period pain. Nurofen is the leading brand. Ibuprofen should be taken with food, and is not suitable for everyone. Check with your pharmacist if you have asthma, kidney problems, or a history of stomach issues.
Combination Painkillers
Co-codamol combines paracetamol with a low dose of codeine (8mg) for stronger pain relief than paracetamol alone. Anadin Extra combines aspirin with paracetamol and caffeine. These are suited to pain that has not responded adequately to a single active ingredient. Codeine-containing products should not be used for more than three consecutive days without medical advice due to the risk of dependence.
Topical Pain Relief
Voltarol (diclofenac) gel is an anti-inflammatory gel applied directly to the skin over the painful area. It is effective for localised muscle and joint pain with fewer systemic side effects than oral NSAIDs. Biofreeze gel uses menthol to provide a cooling sensation that can help relieve muscle and joint pain. Deep Heat products provide warming relief for muscular aches and stiffness.

FAQ
Can I take paracetamol and ibuprofen together?
Yes, paracetamol and ibuprofen can generally be taken together by adults, as they work through different mechanisms. However, do not take ibuprofen with other NSAIDs (such as aspirin or naproxen), and always check that other medicines you are taking do not already contain paracetamol. If you are unsure, ask our pharmacy team.
How long can I take codeine-based painkillers for?
Over-the-counter codeine products (such as co-codamol 8/500) should not be used for more than three consecutive days without medical advice. Regular use beyond this can lead to physical dependence. If you need ongoing pain relief, speak to your GP about a more appropriate long-term treatment plan.
When should I see a doctor about pain?
See your GP if pain is severe or getting worse, has lasted more than two weeks, is accompanied by unexplained weight loss, numbness, weakness, or fever, occurs after an injury and is accompanied by significant swelling or inability to bear weight, or if you find yourself needing to take painkillers regularly.
